How high you are gripping: holding at about 13'8" or thereabouts, a little below the weight sticker.
What bar(s) you are attempting in the video: 13'6" clear
My left arm sucks, straight up, thats one thing I know i need to fix, what other things do you guys see i'm doing horribly wrong?

It seems to me like you are overgripping a bit. It seems like you wouldnt have gotten through on the jump if you hadnt come out early. You could work on swinging your shins to your top hand better, and moving up the pole rather than kind of coming out early. I think lowering your grip 3 inches or so will allow you easier penetration and give you the chance to come back all the way. Just make sure you hit that take off hard with a lot of impulse and keep that top hand from coming so far behind your head. If you keep it slightly behind your head it will smooth up a lot of your jump. Good luck!

I think my step being so in caused me to be so far in front of my top arm
Right you are! Moving your step out 4-6" should do wonders. You are being pulled off the ground instead of being able to jump off the ground. Also it would help to move your arms forward and up as you plant and not just up or up and back. You can then put much more energy into the pole by resisting, i.e. pushing the pole with both arms. This will get the pole moving and will put yo in a much better position on top.
Run fast and go high.
