The first jump is 15'9 I am holding 14'8 one a 16ft 16.5
The second jump is 16'9 I am holding 15'1 on a 16ft 15.6
The third one is 16'10 I am holding 15'1 on a 16ft 15.6
The forth jump is 17'1 I am holding 15'2 on a 16ft 15.6
The fith jump is 17'9 I am holding 15'2 on a 16ft 15.6
the sixth jump is 17'9 I am holding 15'3 on a 16ft 15.2
Just to quickly state the obvious flaws I have to wide a grip and am taking off under. I have many other flaws but think these are the main ones.

I think you're losing a lot of speed and efficiency at take-off because of how far your top hand comes behind you at take-off. You've already mentioned your wide grip (leading to some blocking) so there's no need to mention that. Just keep your top hand tight to your body and drag it up your body during the duration of your pole drop. Also, I think you know where I stand on your swing (I of course it should be a straight whip through the entire vault), but that's a big change so that's your call. Looking good though brotha. -6P

I definitely agree about the top hand. I bring it further behind me every time I start to feel a little bit unconformable. That's why on my first video where I am on a small pole I do not do this very much. Also I think me bringing my top hand behind me is the main thing that cause me to over stride and sink a little into the take off. (I like the way I am swinging, though I still constantly debate on weather to lengthen my swing or not.)

6P and yourself are right about the top hand coming from behind your head during the plant. I think that it may be because you have a low pole carry and are balancing the weight of the pole with your hand behind your hip.
I am not suggesting that you change anything at this point in the season, but think about it for the future. Look at Geo's pole carry as an example to copy.
