Not sure why you are only going from 5 lefts (injury?) but 15' from 5 lefts is excellent!
You need a higher pole carry throughout your run, even from from 5 lefts. It also seems you got hammered a bit at takeoff which cause your left arm to partially collapse.
A nice jump though, I really liked your finish just before you got off the pole. Good extension.

Yea i think the pole drop will fix a lot of things, just because you look very low off the ground. Thats why your pole speed essentially stopped and you had to get off the pole early to make the bar. A good drop and a focus on jumping up on take off will give you a 16' PR from 5.
