By putting it "on the ground", I'm sure you meant "in the box".

Your arms must go up BEFORE the pole hits. You need to start your plant earlier than 2 steps away, so that your top arm is already "up" before the pole hits.

I'm sure that your follow-up question will be "How many steps away from the box should I begin my plant?". I'm going to leave that one for someone else to answer. ;)

That depends on how you define 'plant'. ;)

The plant starts as soon as you begin your run, 3 lefts out from the box, or 3 steps away.

Practice your 'plant' from 3 lefts (so in this sense, I mean the transition from pole carry to pole 'drive').

Take Beccas advice and buy "Beginner to Bubka" -and the dvd as well. A picture is worth a thousand words -and as far as I can see the thousand words so far on this topic have done little but confuse you. ;)
