Do some flyaway work, it looks like your arms are dropping instead of your chest hollowing out and your arms coming up and back.
Your bottom arm collapses at your plant, focus on jumping up into the pole with a stronger set of arms.
Turn around the pole to elevate the hips higher instead of turning out and over the cross bar. To do this full your toes in toward you during your turn and keep your chin tucked.
Other than that, nice jumping, keep workin.

What is nice about this sport is that I am responsible for most everything.

That doesn't mean that he can't still coach you to 16'+... ask altius what HIS pr is... not being able to jump well doesn't mean he can't coach well... just a thought.
What I saw in your jump was a little lack of drive knee, and I agree with the above posts.

Sometimes the best coaches can't do the event at all. They just don't have the physical strength to do it, or for other reasons. Look at Bill Falk, there's no way he vaults good nowadays... I don't know if he ever even vaulted. But he understands how it works, and he can make the vaulters understand.
Have an open mind. Don't get too cocky.
