Help! I need a list of firsts1

I know I've seen it on here somewhere, but my attempts at searching did not help. I'm looking for a list of the names of the first men & women to clear the bar at 12', 13', 14', 15', 16', 17', 18', 19', & 20'. I know most of the men's names, but I'm a little hazy on some heights. (Warmerdam at 15', Uelses at 16', Pennel at 17', Bubka at 20'. Any help would be appreciated. Thanks
