you are getting stuck at the top of the pole because you arent swinging all the way back, because you are staring at the bar. Think about once your feet are as far back as possible, drop your shoulders all the way to the pole. its ok to lose sight of the bar because if you do this you will clear it by a foot anyway. Just stay back on the pole as long as you can, holding that position. Then once it starts recoiling start closing it off, the turn will come naturally.

Post a video (preferably in QuickTime) that I can look at each frame. There are some things you are doing well, but I need to analyze a lot of frames before I can give you some advice.

Hi Pool Man:
Doing some good stuff on your vault. A little tough to see, but it looks like your takeoff is pretty solid. Your top hand never passes beyond vertical, which is really good.

Your primary problem is that you are looking at the bar way too long and are not continuing to swing up on the pole. As your hips pass horizontal, you need to engage shoulder muscles and drop your shoulders to line up, and come around the front of the pole. You are always past the pole, so your natural tendency is to flag out at the top. Look at this link

The athlete in the bottom two videos is continuing to use her shoulders to get to the pole. Notice that her backside is beginning to show, which means that she is already naturally starting to rotate around the pole. Look at this link:

This athlete is learning how to rotate around the pole and stay vertical.

There are other links that will give you the same idea what I am talking about.
Good luck
