Couple things, 1. you drop your left hand on the run before you plant the pole. This will always cause you to take a hit at take-off. You need to keep your left hand high during the approuch, and punch it up. 2. You tend to run into it at take-off, and poke your left hand forward. That again will cause you to take a hit. You want to punch your hand up and get the pole tip to come back towards you as you jump up. You do not want the pole tip to poke out at the back of the box.

Now if you work on those two things your take off will improve and you will be able to get you left leg back more which will then improve you swing, and then you will be able to fix some more stuff.
