On the positive, I'm very impressed with your approach run. Awesome form!
On the negative, your plant is terrible. Fix that plant and you'll be skying some nice heights. Here are some freeze frames I took to compare yourself to:
#1:
You're pushing your plant way out in front of yourself instead of straight up. You're dead already because of this. On the positive, your body is nice and upright.


#2:
You must always jump right at the pole, never off to the side of the pole. A high pressed out plant will insure pole separation from your head/face.


#3:
Head and chest have to move through the "window" with the pole right in front of you.


Summary:
You're bleeding away 90% of that fantastic approach run energy with that terrible plant and take-off. The good news is your approach run is better than most high school athletes. My guess is you are doing this same bad plant on your 3-Left jumps. That's the place to fix it. When straight poling for 3-Left jumps, you must focus on jumping right at the pole with a high pressed out plant with the top arm and the bottom arm elbow bent way out to the left creating the "window" for your head and chest to move through. You won't hit the pole with your face, trust me (as long as you press high)
