You have more pressing problems than those you mentioned...
Most importantly, I believe, is that you're taking off under by at least a foot. MUST FIX THIS!
Then, a close second, you don't hit the chord of the pole long and strong with your trail leg.
And yes, you could benefit from keeping your hands higher throughout your vault, and remember not to 'row' until your hips are above your hands!
