Just remember that a 14' 140 is a good deal bigger than a 13'6" 135. You do have good penetration into the pit but you aren't blowing through the pole. Try to start turning around the pole as opposed to swinging to your top hand and turning from there. If you turn around the pole you will start to come off vertically instead of horizontally over the bar. Look for a transition pole if you have one and hopefully the info helps.

After you take off you swing till you end with your trail leg at your top hand and then you push your legs out and over the cross bar. The majority of your body is between the pole and the cross bar. Take a gander at some of the vaults by Tim Mack or Tommy Skipper or any of the other great vaulters on Youtube and you'll see that when they swing to the top hand, they "pull turn" i.e. they elevate their feet vertically pulling their hands to their hips and turn around the pole, and when they push off they are doing so vertically, off the top of the pole instead of horizontally. Its a tough tough thing to do though, don't get discouraged. Lots of apparatus work, you'll get there.
