How long you have been jumping: 8 years
What pole you are on (length and weight rating): 16'1" 185
How high you are gripping: 15'1"
What bar(s) you are attempting in the video: 15'9"

It should be obvious that you aren't swinging up with a high drive knee and low, straight trail-leg. That is so much more important than people think. I couldn't really see much else too wrong, but then again, it was tough to see anything. Could you get a better video to post?

Im not sure if it was just an illusion from the low framerate of the vid or not but.... beyond what powerplant said, it looks like you could definitely do a better job dropping your shoulders. I don't mean just moving them back, I mean like, thrusting them back. If you do that, you'll put yourself in a much better position to get an efficient push off.

whats going on Zach

one main thing you could focus on is your approach, you have good speed bulding up to your last three steps, then you can see yourself lean back and lose a lot of momentum which dropped you back and stalled you out over the bar. you had tons of height on it, just make sure your last three steps are your strongest in the run while maintaining your speed and you should be blasting over that bar
