This is me jumping on a 14'-6" 165 with standards at 30' and bar is at 13"-11 1/2" holding at 13'-7"
I weigh 150lbs, 16 years old and am in my second year of pole vault if any of that helps.

Your shift was a little late and you were under. Also you pretty much ran into your plant instead of jumping into it. This way you force a big low bend on your pole. It doesnt seem to be a problem for you since your standards are at 30". But moving your step out a little and jumping up more at takeoff will help you move up poles. A trick that i found this summer that helped me raise my grip in sand vault is as your about to plant try to jump over the box/plug of your pole right before your pole hits the box.
