I am not a big fan of giving advice over PVP, but I feel like some people on here are not seeing the underlying problem and possibly adding to your difficulties.
I am not going to pretend that I can fix your vault and guarantee a PR from over the internet, but I can tell you that you are doing alot of things well, yet you are hampered by a problem that I constantly deal with.
There has been talk about your drive knee and not finishing your takeoff, which is true but this is action reaction. Slow down your video. Then play it again in full speed. Listen to the rhythm of your run and watch how your center of gravity moves at the plant. You are too far away, striding out the last few crucial steps, and AS A RESULT, your knee is down and takeoff is incomplete.
If I remember correctly, I believe Brian (VTechVaulter) explained this a few weeks ago. Move your run in 12-18" and keep a nice relaxed posture coming in. Focus on quick, rhythmic steps, ending in a quick, powerful action/reaction takeoff with knee up. If over time you feel too close, slowly back away a few inches at a time, but remember to keep the steps accelerating and powerful.
Focus on that, and I can ALMOST guarantee you will be happy with the results.
